What is the Patient Document Portal?
The Patient Document Portal lets you electronically sign consent and other forms that your dental office needs completed before your treatment using a secure, HIPAA- and PCI-compliant site. Just enter your portal ID, zip code, and last name to access to the forms you need completed. For your protection, your forms will expire after 30 days.
Why should I use the Patient Document Portal?
Our patient’s safety is our first concern. To do everything we can to minimize your in-office time, we use the Patient Document Portal to finish up your “paperwork”, BEFORE you come into the office.
How do I enroll into Patient Document Portal?
Easy, if your dental office has activated the Patient Document Portal, no action is required on your part to enroll. Simply check your text or email for the Portal ID and enter your information here to see if you can sign your forms online.
What is a Portal ID?

How secure is my personal information and my Patient Document Portal documents?
All transactions are processed through a secure website, and all information is encrypted.
Is there a fee associated with the Patient Document Portal service?
No, there is no processing fee charged to patients for using the Patient Document Portal.
Can I add documents that I want to eSign myself?
No, you can only eSign documents that your dental office requests you to eSign. You cannot add forms that you want to eSign on your own.
How long do I have to eSign the documents?
You have 30 days to eSign the documents. If you missed the deadline, you can’t eSign that document. Your office must send you a new request, even for the same document.
The address you have on file for my account is incorrect. What do I do?
If your mailing address has changed or is listed incorrectly, please contact your dental office with the correct information so they can update your file.
Can I see what documents I’ve eSigned in the past?
Yes, the portal gives you access to all the documents you’ve eSigned through this portal. It does not have documents you’ve signed in your dental office. Only those documents you’ve eSigned through this portal.
